NAATs such as RT-PCR for SARS-CoV-2 are designed to detect viral RNA. A positive result is highly specific for the presence of viral nucleic acid; however, it does not differentiate between viable and nonviable virus. Thus, a positive test does not necessarily indicate that a person is infectious and requires isolation.
Thus, they could be moving away from this test because they can’t tell if someone is truly infectious towards others. If the Antigen/Serology tests can do that then I’d be all for it as it would give us a better idea for Positive Cases that are ACTIVE/Infectious.
Most state laboratories in the United States are approved to test for MERS-CoV by using an rRT-PCR assay developed by CDC. This test is done under authority of an Emergency Use Authorization because there are no FDA-cleared/approved tests available for this purpose in the United States.
That you will use any information to determine your outcome. In the case of MERS testing, they don’t need to test for MERS right now so why have the FDA pay for testing.
Viable vs nonviable: taking a sample from someone and introducing it into the world means UV light hits it and could cause it to be a nonviable virus. Viable = living and able to replicate. Non-viable means it was living at one point (thus if the sample was taken from a nose then it is pretty safe to assume that the person HAD the virus or COULD BE positive now). Viral load is important to understand as well.
A sample with very low viral load AND nonviable would likely mean the person is not infectious.
A sample with very high viral load AND viable would likely mean the person is very sick.
Since the PCR can’t determine the viral load of the sample, we have no way of knowing if the person is infectious to others.
